Hiring a licensed Physical Therapist (PT) to provide full-time care at our trusted partner facility, The Gardens at Stevens in Stevens, PA. This is a meaningful opportunity to guide residents through rehabilitation, restore mobility, and help them reclaim their independence all within a supportive and collaborative environment.
What You’ll Do:
Evaluate patients and develop personalized physical therapy treatment plans
Deliver evidence-based, patient-centered interventions
Track progress, update goals, and collaborate with the interdisciplinary team
Ensure services meet Revital’s high standards for quality, documentation, and ethical care
Qualifications:
Licensed Physical Therapist (PT) in Pennsylvania
Current BLS certification
Strong clinical judgment, communication skills, and patient advocacy mindset
