We are hiring for Per Diem, Part-time and Full-time employees!!!
Seniority Healthcare is seeking Nurse Practitioners or Physician Assistants to join our team in Long Island, NY. In this role, you will provide comprehensive care to a high-need, high-risk geriatric population. The ideal candidate has a background in Internal Medicine, Family Medicine, or Geriatrics, and can work independently while collaborating with a multidisciplinary team.
Key Responsibilities
- Conduct thorough physical exams and wellness assessments for seniors
- Follow up on care plans to ensure the best possible outcomes
- Propose and manage treatment plans for chronic and acute conditions
- Maintain accurate patient records and manage appointments effectively
- Educate patients on preventative care, treatments, and healthy lifestyle choices
- Collaborate with a supportive team dedicated to holistic care
- Follow up on email correspondence at home.
Qualifications
- Completion of an accredited NP or PA program
- At least one year of experience in geriatrics, internal medicine, or family practice
- Strong communication, active listening, and analytical skills
- Compassionate and committed to providing patient-centered care
- Motivated to serve a geriatric population with complex care needs
