Job Summary
We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) with home health experience to join our compassionate care team. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation to develop tailored care plans.
- Administer medications, injections, IV infusions, and catheter care following strict aseptic techniques and safety protocols.
- Utilize Kinser software for documentation of patient visits, treatment plans, and medical records management with precision and compliance.
- Conduct diagnostic evaluations such as specimen collection and processing, ensuring proper handling and timely reporting.
- Manage complex cases involving chronic conditions like Alzheimer's disease, dementia, pain management, and infectious diseases within a home setting.
- Educate patients and their families on health maintenance, medication administration, nutrition, and safety measures to promote optimal recovery and well-being.
- Coordinate with multidisciplinary teams for case management, ensuring adherence to long-term care regulations and HIPAA confidentiality policies.
Skills
- Valid LVN license with current certification to practice in the state of employment.
- Proven home health experience with knowledge of Kinser software for electronic health records (EHR) management.
- Strong understanding of ICD-10 coding standards for accurate documentation and billing processes. Check eligibilities for both medicare , medicaid and H.M. O. Patients
- Experience working with diverse populations including geriatrics, individuals with developmental disabilities, memory care needs, and acute or chronic illnesses.
- Proficiency in medical procedures such as IV insertion, phlebotomy, tube feeding, airway management, catheterization, and basic life support (BLS).
- Familiarity with EMR/EHR systems like Epic or Cerner is preferred for efficient clinical documentation.
- Excellent patient assessment skills combined with compassionate caregiver communication to foster trust and compliance.
- Knowledge of infection control practices and long-term care regulations to ensure safe home environments.
