Your Role as a Nursing Supervisor:
- Lead a Team: Supervise and mentor Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to deliver top-notch care.
- Develop Care Plans: Conduct assessments and create personalized care plans tailored to each participant’s medical, emotional, and social needs.
- Train Caregivers: Provide training, monitor performance, and ensure compliance with program policies and state regulations.
- Collaborate Across Disciplines: Work with an interdisciplinary team to address changes in participants’ health or care needs.
- Promote Community Awareness: Participate in outreach efforts to educate families and the community about the AFC program.
- Document with Precision: Maintain accurate and timely documentation to meet program requirements.
What You Bring to the Table:
- License: Active Registered Nurse (RN) license in Massachusetts (required).
- Experience: At least 2 years of nursing experience in home care, adult day health, or long-term care. Supervisory experience is a plus!
- Skills: Strong clinical assessment abilities, excellent communication, and leadership skills.
- Knowledge: Familiarity with AFC/GAFC guidelines, state regulations, and community resources.
- Education: Bachelor’s or Associate’s degree in Nursing preferred.
