Job Summary and Qualifications
As a Licensed Practical Nurse (LPN), you are a valued and important key to success in serving superior patient care. Under the supervision of the Registered Nurse you will provide skilled nursing visits, help create, carry out and evaluate the plan of care for each of your clients. We are looking for a team player who is eager to learn, passionate about providing quality care, and has a proven track record of compassionate care.
What you will do in this role:
- You will provide skilled nursing care under the direct supervision of the Registered Nurse
- You will assist the Registered Nurse in carrying out the plan of care, and will help with the preparation, implementation, and continuity evaluation of the client care plan
- You will assist the Registered Nurse in performing specialized procedures
- You will help teach the client the appropriate self-care techniques
- You will observe, record and report to the appropriate person the general physical and mental condition of clients and signs of symptoms which may be indicative of changes in condition
- Further assist in rehabilitation by reporting any abnormalities in the client’s range of motion, body mechanics or body alignment
- Actively participate in weekly case conferences, inservices, and any staff meetings
Qualifications
- You must hold a current LPN license that is in good standing with the state board of nursing
- You must have a minimum of one year experience as an LPN
- Home care experience will be prioritized
- You must be able to provide two references
- You must be able to pass OIG checks, Virginia State Police background check, and national background checks, and able to verify employment eligibility with necessary documentation
- A current CPR certificate is required
- Reliable transportation, proof of valid car insurance, and a valid driver’s license
