TITLE: PATH Outreach Specialist
REPORTS TO: Outreach Director
POSITIONS SUPERVISED: None
CLASSIFICATION: Full Time Hourly
DUTIES:
1) Identify PATH eligible individuals and establish consistent PATH client follow-up activities.
2) Assist Director and Outreach Team in establishing and maintaining a professional rapport with community organizations that serve the homeless, mentally ill, those with other severe mental health disorders, and those with co-occurring disorders of substance abuse and mental illness. Provide education about the PATH
Program/MVHO to consumers, referral agencies, and associates.
3) Assist Director and Outreach Team in making regular visits to emergency shelters, feeding sites, and sleeping sites to meet homeless persons. Identify those who exhibit apparent signs of severe mental illness or severe mental illness and substance abuse problems. Provide assistance in the form of vouchers, food, clothing, and immediate access to temporary housing. Collect, document, and maintain accurate and timely information and reports about potential and/or enrolled program participants. Complete and provide appropriate case notes and documentation of all meetings/services provided to potential and/or enrolled program participants as instructed by the Director.
4) Assist Director and Outreach Team in linking individuals with mental health, substance abuse, housing, and other community service providers. Provide program participants with outreach services, encouragement, support, and assistance in applying for and obtaining services and benefits. Learn and follow appropriate eligibility procedures for engaging clients in these services/programs.
5) Assist Director and Outreach Team in responding to requests for urgent assistance. - Be available to respond on short notice at odd hours to requests for support and assistance from enrolled or potential participants, Gateway shelters, or local safety personnel.
6) Comply with MVHO’s Personnel Policies and Procedures Manual.
7) Perform other duties as assigned. Attend trainings and other personal/professional development opportunities, as assigned and planned with supervisor.

Miami Valley Housing Opportunities (MVHO) is seeking candidates for a Projects for Assistance in Transition from Homelessness (PATH) Outreach Specialist position. The full-time employee will work in an outreach team and with local law enforcement and emergency housing providers to identify mentally ill homeless persons living in places not meant for habitation in Montgomery County, Ohio; provide services and link them with behavioral health care, substance abuse, housing and other services provided in the community. They will also complete reports, logs of activities and document services/contacts on program databases, such as the Homeless Management Information System (HMIS).
Preference is given to formerly homeless applicants recovering from mental illness
/ substance abuse disabilities. A completed education is preferred but not required in Mental Health Technology, Social Work, or another related area; some advanced
education in at least one of these areas is required. Previous experience preferred in case management, social service, housing service, or related area. Familiarity with the state and local mental health service community is preferred. Candidates must have computer skills including word processing, calendar and email management and data entry accuracy. Employee must have and maintain reliable personal transportation. Must be willing to work odd and irregular hours and be capable of working productively with minimal supervision. Must also be capable of walking during all or most of an eight-hour shift and/or be capable of remaining outdoors in most weather conditions during all or most of an eight-hour shift.
Must pass a criminal background inquiry and be willing to work closely with law enforcement officials and business owners to address their concerns, as well as assist homeless participants.
