License/Certification Requirements: Valid LPN License from the State of Minnesota Board of Nursing; CPR/BLS Certification and maintenance. Valid MN Driver's License and current insurance.
Summary: Under the Supervision of the Director of Nursing, the Clinic LPN will provide primary health to service area residents in community clinics. Also responsible for cultivating relationships with the community and accessing the needs of the clinic.
Education Requirements and Experience:
Current LPN, with at least 2 years in primary health care or a Current LPN with preferred 2 years' experience in clinic work.

Ability to assess, plan, implement and evaluate nursing care according to individual needs.
Must demonstrate an understanding of clinical risk.
Must demonstrate consistent professional conduct and meticulous attention to detail.
Must possess excellent verbal and written communication skills as well as excellent interpersonal skills with patients, staff, and other health care professionals.
Excellent time management and organization skills required.
Ability to establish and maintain effective working relationship with staff, clients, and other professionals.
All licensing renewal requirements as specified by the MN state Board of Nursing, including required CEU's.
Familiarity with the Leech Lake Reservation and surrounding communities is preferred.
Experience in utilization of Electronic Health Record (EHR) technology or willingness to learn.
Native American preference.
Must meet Health Division hiring guidelines.
Pre-employment drug testing is mandatory for Health Division positions.

Duties/Responsibilities:
Assists provider with patient office visits, including rooming patients, clinical interview, recording vital signs, documentation in EHR.
Administers injections.
Assists RN with clinic support and guidance.
Performs select clinical tests in scope of an LPN.
Performs administrative tasks, including form completions, prescription refills, referrals and patient educational material.
Administers and documents medication under physician's order and follows the five (5) medication rights and reduces the potential for medical error.
Fields patient telephone calls and responds accordingly.
Organizes exam rooms, including general cleaning, upkeep, and re-stocking of supplies.
Utilizes Fleet or Tribal vehicles daily, personal vehicles only allowed with manager or director of nursing approval as needed.
Where assigned will partake in Health Division events, programs and initiatives.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Provides high quality health care services under the direction of the clinic provider.
Provide individualized nursing care to patients.
Able to access and assist with health services
Education of and effective communication with those served concerning the diagnosis and treatment of their medical conditions, appropriate preventative measures and use of the health care system.
Utilizes appropriate age and population standards.
Cares for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow up as patients move along the continuum of care.
Utilize the Reservation community resources.
Participate in community education and other professional activities.
Attend and or conduct all mandatory meetings and trainings as required.
Maintain confidentiality and professional ethics at all times.
Travel in and out of the state as needed
Understanding and sensitivity to Native American culture.
Adhere to policies governing staff conduct.
All other related duties as assigned.

May require driving in sub-optimal weather and road conditions.
