Job Title: Community Care Department (CCD) Manager
In-person position : M-F, 8AM-5PM
For 30 plus years, Oxford HealthCare has committed to excellence in providing home care services to the elderly and disabled in Tulsa, Oklahoma and the surrounding areas. Oxford's Community Care Department (CCD) provides Grant funded, in-home services that support the independence and quality of life of the aging population throughout Tulsa, Creek, and Osage Counties. Oxford is in need of a Community Care Department Manager to plan, organize, coordinate, direct and evaluate the services provided within the Community Care Department of Oxford HealthCare, as well as serve as first contact for clients, family, and referral sources.
Position Summary:
Position includes assigned responsibilities involving the direction or supervision of programs related to the delivery of agency services grant, funded under the Older Americans Act of 1965. Supervision of professional level staff in providing a variety of services and assistance to eligible clients, responsibility for work groups throughout grant service department, assisting in directing major program implementation, and assisting in planning/organizing of day-to-day operation with the agency.
Responsible for monitoring and compliance with all budgeted targets. Complies with all state licensure standards. Provides counseling, case management, and advocacy to help people in need of assistance. Serves as the Pathfinder by being a first point of contact for clients, families, and referral sources, combining a strong sales-driven mindset with a compassionate, solutions-oriented approach. Guiding clients toward appropriate care solutions within the services provided by Oxford HealthCare (OHC) or providing community resources.

Education/ Experience:
- Bachelor's degree in social services related field with one (1) year of experience in hospital, nursing home, or home care setting.
- Experience and knowledge with Federal Grants, as well as grant writing and proposals.
- Social worker / case manager experience preferred
