About CrossOver Healthcare Ministry: CrossOver Healthcare Ministry provides compassionate, high-quality healthcare to the uninsured and medically underserved in our local community through the operation of two healthcare clinics; one in the City of Richmond and one in Henrico County. CrossOver is a 501(c)(3) non-profit organization that offers comprehensive healthcare services to its patients, including: primary care, specialty care, pediatrics, dental, vision, behavioral health, HIV/AIDS care, women’s health & OB/GYN, medications and medication-management, and social work case management.
Position Summary: The Nursing Coordinator – Richmond Clinic will support CrossOver’s clinical team of staff and volunteer providers by overseeing nursing staff practice, training, and education at the Richmond clinic.
Hours: 40 hours per week. Generally, 8:30 a.m. – 5:15 p.m., but some evening and weekend work will be required. This position will be located at CrossOver’s Richmond Clinic at 108 Cowardin Ave.
Salary: 60K-70K depending upon experience

Key Responsibilities:
- Provide care coordination, referral and follow-up to individuals and families who are members of a vulnerable population and/or high-risk groups.
- Evaluate outcomes, effectiveness of care plans and make changes as necessary.
- Provide education and counseling to individuals, families and community groups that are adapted to their unique needs, lifestyle, cultural and socio-economic status.
- Collaborate with community partners to promote the health of the population served.
- Oversee nursing practice (of volunteer and employed staff) at CrossOver’s Richmond Clinic in the primary care program.
- Supervise primary care nurses, MAs, and students. Train new volunteers and employed nurses. Manage nursing schedule.
- Manage and assist in complex patient care cases and care management.
- Oversee vaccination inventory and clinic supplies, with the help of the clinical coordinator.
- Participate in nursing steering committees and clinical leadership committees.
- Implement practice changes as needed on the clinic floor. Participate in qualitative improvement initiatives.
- Oversee miscellaneous clinic administration tasks.
Key Qualifications:
- Registered nurse with experience in community health and strong interpersonal skills.
- Strong interest in promoting wellness to the underserved community.
- Knowledge of emergency preparedness at work and in the community
- Demonstrated ability to work collaboratively with multicultural and multidisciplinary teams to improve patient outcomes.
- Demonstrated ability to maintain working relationship with diverse populations and cultures; ability to engage patients and families in plan of care.
- Strong critical thinking/problem solving skills, as well as ability to prioritize multiple tasks with great attention to detail and logistical management.
- Ability to communicate effectively, both orally and in writing.
- Knowledge of computers and various software applications.
- Proficiency in Spanish preferred
