Responsibilities
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs and reporting any changes to healthcare professionals promptly
- Administer medications and perform catheter care following medical instructions and home healthcare regulations
- Maintain accurate medical documentation using Electronic Health Record (EHR) systems and ensure compliance with HIPAA confidentiality standards
- Support clients with meal preparation, light cleaning, laundry, and other household tasks to promote a safe and comfortable environment
- Utilize patient transfer methods including Hoyer lifts and proper patient transfer techniques to ensure safety during mobility assistance
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care strategies and behavioral management
- Assist with end-of-life care in hospice settings, offering emotional support and ensuring dignity for patients and families
- Follow infection control procedures, including hand hygiene and use of personal protective equipment (PPE), to safeguard client health
- Communicate effectively with clients, families, and healthcare teams to coordinate care plans and update progress regularly
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred
- Demonstrated knowledge of medical terminology, medication administration, and basic life support (BLS) or CPR certification required
- Experience working with individuals with developmental disabilities or geriatrics enhances your ability to provide empathetic care
- Familiarity with EMR/EHR systems for documentation purposes is advantageous
- Proven ability to perform heavy lifting safely using proper techniques or equipment such as Hoyer lifts or patient transfer methods
- Previous live-in employment experience or long-term caregiving roles are beneficial
- Knowledge of home & community care regulations ensures compliance with industry standards
