Nurse Practitioner (NP) / Physician Assistant (PA) – Home Visiting Provider (House Calls)
We are seeking a compassionate and independent Nurse Practitioner (NP) or Physician Assistant (PA) to provide in-home medical care to patients in their residences. This role is ideal for a provider who enjoys working autonomously while delivering high-quality, patient-centered care outside of a traditional clinic setting.
Providers will travel between patients' homes throughout the assigned service area, performing comprehensive medical evaluations, chronic disease management, acute care visits, and follow-up appointments.
Responsibilities
- Conduct in-home medical visits for adult and geriatric patients.
- Perform comprehensive physical examinations and assessments.
- Diagnose and treat acute and chronic medical conditions.
- Order and interpret laboratory tests and diagnostic studies as appropriate.
- Prescribe medications and treatments within state scope of practice.
- Develop and manage individualized treatment plans.
- Document visits accurately in the electronic medical record (EMR).
- Coordinate care with primary care physicians, specialists, home health agencies, and family members.
- Educate patients and caregivers regarding disease management and preventive care.
- Maintain timely communication with the supervising physician and clinical team when necessary.
- Travel independently between patient homes throughout the designated coverage area.
Qualifications
- Current, unrestricted NP or PA license.
- National certification as an NP or PA.
- Current DEA registration (or ability to obtain).
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability and willingness to drive between patient homes throughout the workday.
- Strong clinical assessment and decision-making skills.
- Excellent communication and documentation skills.
- Experience in primary care, geriatrics, internal medicine, family medicine, home health, hospice, or mobile medicine preferred.
Schedule
- Flexible part-time opportunities available.
- Monday-Friday
- No office time required; providers travel directly to patient homes.
Compensation
- Paid per completed patient visit (per-patient compensation).
- Compensation is based on the number and type of patient visits completed.
- High earning potential based on productivity.
- Mileage reimbursement (if applicable).

Pay: $600.00 - $850.00 per day
