As a Physical Therapist (PT), you will provide skilled therapy services to patients in their homes. You will develop and implement treatment plans that restore function, improve mobility, and promote overall well-being, while educating patients and families to ensure continuity of care.
Responsibilities
- Conduct thorough evaluations of patients' physical abilities and limitations.
- Develop and implement personalized treatment plans based on patient assessments.
- Provide therapeutic interventions such as exercises, manual therapy, and modalities to promote recovery.
- Educate patients and their families about treatment options, home exercises, and injury prevention strategies.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Document assessments, progress notes, and care plans promptly in our EMR system (Axxess).
- Participate in discharge planning to ensure smooth transitions for patients returning home or moving to other care settings.
- Maintain up-to-date knowledge of medical terminology and best practices in physical therapy.
Qualifications
- Licensed Physical Therapist in Pennsylvania.
- Minimum of 1 year of experience in home health or outpatient settings preferred.
- Knowledge of EMR systems (experience with Axxess is a plus).
- Strong interpersonal, organizational, and communication skills.
- Ability to work independently and manage a flexible schedule.

Pay: $50.00 - $80.00 per hour
