Current RN or LPN license is required for this position.
Assist the Resident Service Manager with the provision of services to residents and the companion staff members.
Assist with the following: Orients, trains, motivates, counsels, disciplines, and oversees the Companion staff. Ensures staff compliance with facility policies, protocols, and safety measures. Monitors and ensures staff compliance with documentation.
- Counsels residents and/or their families in meeting the residents’ care needs.
- Monitors the health of each resident and notifies the appropriate person or agency of changes in condition or needs.
- Ensures departmental compliance with facility safety and Quality Assurance Programs.
- Collaborates with members of the management team to ensure residents’ needs are met.
- Performs all other duties as required, consistent with the education and skills required for this position.
- Assist the Director of Resident Care Services with the evaluation and assessment of potential residents.
- Direct Care Floor nurse a minimum of 2 days per week.
- Performs evaluations / assessments of potential residents.
- Identifies personal care needs of potential residents.
- Educates potential residents and their representatives of available services.
- Completes all admission assessment documentation.
- Participates in the assessment review process.
- Enters assessment data into the electronic service record program.
Assist the Director of Resident Care Services with all aspects of the Group Adult Foster Care Program (GAFC) as well as evaluate and assess potential residents.
- Assists in the evaluation of potential residents / participants.
- Completes all admission assessment documentation.
- Assists the Director of Resident Care Services with all aspects of the GAFC Program including submission of the clinical application, monitoring for program approval, preparation of semiannual updates and requests for physician recertification, completion and documentation of participant visits, development and review of the care plan, preparation of annual MDS assessments and discharge planning tasks.
- Ensures staff compliance with facility policies, protocols, and safety measures.
- Counsels residents and/or their families in meeting the residents’ personal care needs.
- Collaborates with members of the management team to ensure that resident needs are met.
- Ensures compliance with EOEA regulations.
- Performs other duties as required, consistent with the education and skills required for this position.
Pay: $37.00 - $39.00 per hour
