Care with Dignity at Home® is seeking a compassionate, professional, and experienced Registered Nurse (RN) to conduct in-home assessments for seniors and Veterans throughout Spotsylvania, Stafford, and Prince William Counties.

What You'll Do:
- Conduct initial client assessments and reassessments in the home
- Evaluate physical, cognitive, emotional, and environmental needs
- Develop and update individualized care plans
- Communicate assessment findings with agency leadership
- Ensure documentation is accurate, thorough, and completed on time
- Serve as a trusted clinical resource for clients, families, and caregivers

Qualifications:
- Active Virginia Registered Nurse (RN) License
- Minimum 2 years of nursing experience preferred
- Home health, hospice, case management, senior care, or community health experience strongly preferred
- Excellent assessment and communication skills
- Reliable transportation and valid driver's license
- Ability to work independently and professionally
