West Valley Health Equity, DBA, Rooted in Grace Foundation, is a trauma-informed nonprofit organization dedicated to improving the health, stability, and well-being of pregnant and parenting women and their children who are experiencing homelessness, housing instability, substance use disorders, behavioral health challenges, domestic violence, human trafficking, justice involvement, and other significant barriers. Through its innovative Health + Home model, RIG combines safe, health-supported housing with care coordination, healthcare navigation, behavioral health and recovery support, parenting education, life skills coaching, benefits assistance, and connections to community resources. As part of its family-centered services for families with children ages 0–5, Rooted in Grace delivers three evidence-informed interventions: In-Home Parenting Education (Triple P), Healthcare Referral & Navigation, and Screening & Resource Linkage. These interventions strengthen parenting skills, connect families to preventive and ongoing healthcare, identify developmental and family needs through screening, and link caregivers to community resources that promote healthy child development and family well-being. By partnering with healthcare providers, community organizations, and public agencies, Rooted in Grace helps families access prenatal, postpartum, pediatric, primary care, and recovery services while promoting long-term housing stability, family preservation, and self-sufficiency. The organization's holistic approach addresses the social factors that impact health, empowering families to build resilient, healthy futures and break the cycle of crisis through compassionate, coordinated, and relationship-based support.
Member Duties : AmeriCorps members at Rooted in Grace Foundation provide compassionate, non-clinical support to pregnant and parenting women and their children as they work toward stable housing, improved health, and long-term self-sufficiency. Members assist with housing stabilization, care planning, healthcare navigation, benefits enrollment, appointment coordination, transportation, and referrals to community resources. They also provide parenting education, life skills coaching, recovery support, child development guidance, and ongoing encouragement through regular, relationship-based interactions. Working alongside healthcare professionals, behavioral health providers, and peer support specialists, AmeriCorps members help families overcome barriers, stay engaged in essential services, strengthen family stability, and build healthier futures through trauma-informed, family-centered support.
