Luxe Home Health is looking for a dedicated Home Health Registered Nurse (RN) to join our team. In this role, the Home Health Registered Nurse (RN) will provide compassionate, high-quality care to patients in their homes. This is a flexible, per diem opportunity ideal for an experienced RN with a passion for home health care.

Responsibilities:
- Perform comprehensive patient assessments and develop individualized care plans.
- Administer prescribed medications and treatments.
- Monitor patient progress and communicate with the healthcare team.
- Provide patient and family education regarding healthcare needs.
- Complete accurate and timely documentation for each visit.
- Travel throughout the service area to provide patient care.
Requirements:
- A valid IL Registered Nurse (RN) license is required.
- At least 2 years of nursing experience, preferably in home health.
- CPR and BLS certifications are required.
- A valid IL driver’s license, auto insurance, reliable transportation, and the ability to travel throughout the service area are essential.
- Axxess experience is preferred.
