Pay: $28.00–$34.00 per hour
1. Mandatory Requirements to Apply
- Active Florida Licensed Practical Nurse (LPN) license in good standing
- Basic Life Support (BLS) certification required
- Minimum 1 year of nursing experience required
- Hospice, palliative care, home health, long-term care, or end-of-life care experience preferred
- Experience with medication administration, IV infusions, wound care, and patient assessments
- Proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth
- Strong knowledge of hospice care, symptom management, pain management, dementia care, and Alzheimer's care
- Excellent communication and patient education skills
- Ability to work independently and collaboratively with interdisciplinary healthcare teams
- Reliable transportation and flexibility to travel within assigned coverage areas
2. Roles & Responsibilities
- Conduct comprehensive patient assessments and monitor changes in patient condition to support individualized hospice care plans
- Administer medications, injections, IV infusions, and other prescribed treatments while maintaining compliance with HIPAA and infection control standards
- Provide compassionate nursing care focused on comfort, symptom management, pain control, and quality of life for hospice patients
- Educate patients and family members regarding disease progression, medications, and end-of-life care planning
- Document care accurately within EMR systems and collaborate with physicians, case managers, and interdisciplinary care teams
