Cascade Springs is a locally owned and operated Hospice and Home care company. We take pride in providing the best possible care to our patients. We provide services for the frail elderly, developmentally disabled, patients recovering from surgery or illness, as well as those living with long-term disability or illness. The Cascade Springs occupational therapist and physical therapist will provide physical therapy services to patient’s/clients who have been identified as appropriate for Home Health physical therapy. Wage will be on a Visit Rate basis.We are seeking a dedicated and compassionate Physical Therapist and Occupational Therapist to join our dynamic healthcare team. We are interested in Fulltime and Part Time Physical and Occupational Therapist to join our team in the Tooele, Magna, West Valley City UT Areas.In this role, you will be responsible for assessing, developing, and implementing personalized treatment plans to improve patients’ mobility, strength, and overall well-being. You will work closely with a multidisciplinary team of healthcare professionals to deliver high-quality patient care across diverse settings, including outpatient clinics, inpatient hospitals, and home health environments. Your expertise will help patients recover from injuries, surgeries, or manage chronic conditions, ultimately enhancing their quality of life.
Responsibilities
- Conduct thorough patient evaluations to determine physical therapy needs, including mobility assessments and functional capacity testing.
- Develop comprehensive treatment plans tailored to individual patient goals, medical history, and physiology knowledge.
- Implement therapeutic interventions such as manual therapy, trigger point therapy, exercise programs, and modalities to promote recovery.
- Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care and discharge planning.
- Maintain accurate and detailed clinical documentation in accordance with medical terminology standards and clinical documentation standards.
- Monitor patient progress regularly and adjust treatment plans accordingly to optimize outcomes.
- Educate patients and their families on treatment techniques, home exercises, and community resources to support ongoing rehabilitation.
Skills
- Strong knowledge of anatomy, physiology, orthopedics, neurology, pediatrics, geriatrics, and outpatient rehabilitation principles.
- Experience working with diverse populations including geriatric patients, pediatric patients, and those in home health or outpatient settings.
- Proficiency in electronic health records (EHR) management systems and EMR systems for accurate documentation of patient care activities.
- Excellent client communication skills to motivate patients and explain treatment plans clearly.
- Ability to develop effective treatment plans based on clinical assessment findings and patient goals.
- Familiarity with inpatient experience, hospital protocols, outpatient clinic procedures, and outpatient rehabilitation center operations.
- Knowledge of discharge planning processes and working within clinical documentation standards for compliance.

Pay: $46.87 - $56.45 per hour
Expected hours: 24.0 – 40.0 per week
