About the Role:
Responsible for the overall supervision of the care coordinator staff to ensure maximum effectiveness and efficiency in the delivery of services to families and individuals residing in the Emergency Housing Provider Management Program (EHPMP) facilities. The Clinical Director is responsible for supervising overseeing and monitoring documentation in client files related to client assessments, support services, community linkages and permanent housing applications, and referrals for permanent housing placements. In addition, the Clinical Director is responsible for conducting and documenting psychiatric evaluations for clients living in the EHPMP Program to assess stability and obtain documentation for supportive, and/or permanent housing applications.
Responsibilities:
- Reviews case file documentation submitted by case management staff to ensure adherence to HRA Case Management standards and guidelines, ensuring quality, appropriateness, and compliance with regulations.
- Provide clinical supervision and leadership to case management staff.
- Lead clinical audits, incident reviews, and documentation improvement plans.
- Monitor program outcomes and lead data-driven quality improvement initiatives.
- Ensure adherence to evidence-based practices, trauma-informed care principles, and harm reduction strategies.
- Develop and oversee clinical workflows, treatment planning protocols, and crisis intervention strategies.
- Oversee the timely and accurate completion of biopsychosocial assessments, diagnostic evaluations, and service plans in accordance with organizational and funder requirements.
- Conduct clinical case reviews, support risk management, and advise on high-risk client situations.
- Assists with orienting new Housing Case Management staff to the EHP case management program component and to their role within the department.
- Ensure Housing Case Managers assess clients’ needs and link clients to community referrals for supportive services. Oversee the tracking of the linkage log monthly.
- Assists the Program Director with data collection for measurement and tracking of care coordination services
- Assessing the training needs of the Housing Case Managers and making recommendations for professional development
- Monitors entitlements process for clients and ensures that they receive the appropriate level of benefits for which they are eligible.
- Ensure confidential case records are maintained for all clients.
- Ensure Housing Case Managers liaise with HRA care coordinators and that they conduct collaborative efforts with community service providers to maximize service delivery and ensure client access to supportive services including health, mental health, childcare, children and youth services, job development, employment.
- Ensure Housing Case Managers team provides follow-up for referrals
- Performs all other duties as requested by Program Director and other duties as required.
Qualifications:
Education:
LCSW/LMHC degree required.
Experience:
At least one year's supervisory experience is preferred. Excellent writing skills required. Knowledge of the following service areas is required: homeless-related issues, criminal justice, HIV/AIDS, substance abuse. Experience with assessing client’s mental health, utilizing DSM 5 for diagnosis and strong clinical skills required.
Other Requirements:
Ability to travel locally using public transportation. Required to speak clearly and hear the spoken word well. Required on a regular basis to be prepared to utilize appropriate techniques to ensure consumers' safety. Required to utilize near vision to read data and other documents including spreadsheets and reports in printed form or on computer screens. Regularly required to move fingers, wrist, and arms in a repetitive manner to operate office equipment.
Pay range and compensation package:
Pay: $90,000-$95,000 per year
