What You'll Do
- Provide routine and PRN hospice visits in patients' homes, assisted living, and memory care settings.
- Deliver pain and symptom management, wound care, and ongoing nursing assessments.
- Educate and support patients and families throughout the hospice journey.
- Collaborate with physicians, chaplains, social workers, intake staff, and the interdisciplinary care team.
- Complete timely and accurate documentation in the EMR.
What We're Looking For
- Active Minnesota Registered Nurse (RN) license.
- Hospice experience preferred.
- Nurses with backgrounds in Oncology, Palliative Care, Home Health, or Medical-Surgical nursing are encouraged to apply.
- Excellent communication and compassionate patient care skills.
- Ability to work independently in a field-based setting.
- Reliable transportation and willingness to travel throughout the Greater Twin Cities Metro Area.
