Join our compassionate team as a Direct Support Professional at The Graham Center Adult Day Treatment Program and make a meaningful difference in the lives of individuals with developmental disabilities, intellectual disabilities, and physical challenges.
This position is different than providing services in a group home. We provide services Monday through Friday, 7am to 3pm. You will be actively engaged with our individuals in such activities as current events, exercises, computer training, arts and crafts, life skill training, personal hygiene and large social group activities.
You will help empower residents to achieve their goals while ensuring their safety and well-being. This role also involves the personal care of individuals including assistance with eating and toileting.
We are located within a clean and upscale business center which is close to Tyrone Square Mall and surrounded by shops and restaurants. There are lush community parks close by that are visited frequently by us for outings and activities.
Program Implementation & Support
- Actively engage in daily program activities including life skills training, social, recreation, and community-based instruction
- Implement individualized service plans and person-centered goals
- Provide one-on-one and group support based on individual needs and abilities
- Adapt activities and environments to maximize participation and success
- Teach functional skills including communication, social skills, and self-care
Community Integration
- Accompany individuals on community outings to promote social inclusion and generalization of skills
- Facilitate social interactions and relationship-building with peers
Documentation & Team Collaboration
- Complete daily attendance, progress notes, incident reports, and data collection forms
- Track progress toward individualized goals and objectives
- Communicate regularly with families, guardians, and team members
- Attend team meetings, case conferences, and training sessions
Required Qualifications
- Previous experience in working in an adult day training program
- High school diploma or GED required
- Valid driver's license with clean driving record (for community-based activities)
- Ability to pass background clearances from a federal level (criminal, child abuse clearance, FBI fingerprinting) and from the local level
- CPR and First Aid certification (or obtain within 30 days of hire)
- Ability to document daily activities, monthly reports, quarterly reports and arrival and departure times
- Physical ability to assist with transfers, mobility support, toileting, assistance with eating and managing challenging behaviors safely
Preferred Qualifications
- Must have completed high school and graduated
- Associate or bachelor’s degree in psychology, Special Education, Human Services, or related field
- 1+ years experience working with individuals with developmental and /or intellectual disabilities, autism, or physical disabilities in a day program, educational, or therapeutic setting
