Full Time NHTD/TBI Service Coordinator
Looking to hire Service Coordinators in the Rochester Region. Serving counties of: Monroe, Genesee, Livingston, Wayne, and Ontario.
VRD Group Corp is a Conflict Free Case Management Agency providing services to individuals enrolled in the TBI/ NHTD Waiver. The individuals who are enrolled in the TBI/NHTD Waiver have the needs that would qualify them to be in a Skilled Nursing Facility, but have chosen to receive the services necessary while residing in the community. Service Coordination is necessary to help those individuals navigate the system, access new services and assist with keeping the individuals safe within the community.
VRD Group Corp is based in Brooklyn, NY and has expanded into the Rochester Region. VRD Group Corp is looking to hire a Service Coordinator to cover counties in Monroe and surrounding counties. Service Coordinator is responsible for meeting in person with each individual on the caseload at least one time per month, as well as maintain contact with the participants/informal supports throughout the month, renewing plan as necessary, monitoring deadlines, maintaining contact and alerting the Waiver Director with concerns and issues, maintaining individual’s files, coordinating Team Meetings and interacting with professional agencies to advocate for participants.
Minimal Requirements for Service Coordination Position:
Must Possess a Bachelor’s Degree in Social Work, Education, Psychology or Related Field AND at minimum of one year experience providing Case Management Service to individuals with disabilities and/or seniors. Experience must demonstrate linkage and referral of services to community/waiver based resources for individuals with disabilities OR a Master's Degree in Social Work.
The ideal candidate has excellent organizational skills with a high attention to detail, strong verbal and writing skills and is computer literate. Candidates should have the ability to successfully multitask, be able to work independently, but also a supportive member of the team. Knowledge of Google Drive, Epaces, Social Service Organizations and being bilingual are highly desirable. Candidates must have access to reliable transportation as this position requires the Service Coordinator to visit participants in the home and at various locations in the community. The applicant must reside in New York State with the ability to travel within Monroe County and surrounding counties.
