Case Manager

Job Description:
The Case Manager role presents an exciting opportunity to make a meaningful impact in collaboration with the Sterling Heights Fire Department and the community we serve to provide vital support to individuals in need of behavioral health services. Responsibilities will include assessing clients, connecting them to resources, and assisting them in achieving stability. This role will collaborate with the fire department, law enforcement, court systems, and social service providers to ensure comprehensive care that reduces involvement in the criminal justice system and minimize repeat interactions with law enforcement.
The position is employed by Face Addiction Now and is supervised by FAN’s Clinical Director. It will receive daily direction and supervision from the identified supervisor within the Sterling Heights Fire Department.
Key Responsibilities:
· Conducting comprehensive assessments for individuals referred by Sterling Heights personnel.
· Providing case management services to connect participants with healthcare, mental health, and social service resources.
· Collaborating with peer recovery coaches to offer ongoing support and treatment access.
· Facilitating warm hand-offs to services such as health insurance enrollment, housing support, food assistance, and healthcare.
· Developing innovative approaches to support community members effectively.
· Reducing barriers and meeting people where they are in their journey.
· Maintaining regular communication with the Sterling Heights Fire Department and community stakeholders.
· Keeping detailed case notes and data for program evaluation and reporting.
· Participating in crisis interventions and on-call shifts to provide 24/7 support.
· Working collaboratively with a partner Case Manager to ensure continuity of care for participants.
Qualifications, Skills & Experience:
· Bachelor’s degree in Social Work, Criminal Justice, Counseling, Psychology, or a related field (or equivalent professional/lived experience).
· At least 2 years of experience in case management, preferably in behavioral health, criminal justice, or social services.
· A strong understanding of mental health and substance use disorder services.
· Proven ability to work collaboratively with law enforcement , courts, or community health programs.
· Valid driver’s license and reliable transportation.
· Must pass a background check and drug screen within 48 hours of being offered a position.
Physical Requirements:
Ability to lift to 25lbs., and sit, stand, bend, climb, lift, carry and/or reach frequently and for extended periods of time

Reports to FAN’s Clinical Director
Job Type:
Full-time, up to 40 hours per week
Pay:
$25.00 - $26.00 per hour
Schedule:
· 8 hour Shifts
· Monday to Friday
· Weekend availability
