Position Summary
We are seeking a highly motivated, organized, and compassionate Patient Care Coordinator to serve as a key member of our patient care team. This individual will be responsible for guiding patients through their treatment journey, coordinating care, managing patient communications, supporting provider schedules, and ensuring an exceptional patient experience from initial inquiry through ongoing treatment.
The ideal candidate is personable, detail-oriented, proactive, and passionate about helping individuals access high-quality mental health care.
Key Responsibilities:
Patient Engagement & Communication
- Serve as a primary point of contact for new and established patients.
- Respond promptly to phone calls, emails, text messages, and patient inquiries.
- Educate prospective patients about available services and treatment options.
- Assist patients in understanding the intake process and next steps for care.
- Maintain consistent communication throughout the patient's treatment journey.
Care Coordination
- Coordinate appointments, consultations, follow-up visits, and treatment schedules.
- Assist with patient onboarding and intake processes.
- Facilitate communication between patients, providers, therapists, referral sources, and family members when appropriate.
- Ensure continuity of care and timely follow-up for patient needs.
Treatment Program Support
- Support patients receiving medication management, TMS, Spravato, ketamine, and psychotherapy services.
- Monitor patient progress through scheduled check-ins and follow-up outreach.
- Assist with treatment scheduling and coordination of multidisciplinary services.
- Help identify barriers to care and work collaboratively to improve patient adherence and outcomes.
Administrative Responsibilities
- Maintain accurate and timely documentation within the electronic medical record (EMR).
- Verify completion of required forms and documentation.
- Coordinate insurance authorizations and treatment approvals when applicable.
- Manage provider schedules and assist with appointment optimization.
- Track referrals, consultations, and patient conversion metrics.
Referral & Relationship Management
- Build and maintain positive relationships with referring providers and community partners.
- Ensure timely communication regarding referrals and patient status updates.
- Support outreach initiatives designed to improve access to mental health care.
Qualifications
Required
- High school diploma or equivalent.
- Minimum of 2 years of experience in healthcare, behavioral health, medical office administration, patient coordination, customer service, or a related field.
- Strong verbal and written communication skills.
- Excellent organizational and multitasking abilities.
- Proficiency with electronic medical records and standard office software.
- Ability to maintain confidentiality and professionalism in a healthcare environment.
Preferred
- Experience in psychiatry, psychology, behavioral health, or medical specialty practices.
- Familiarity with insurance verification and prior authorization processes.
- Experience working with treatment programs such as TMS, Spravato, ketamine therapy, or other specialty services.
- Knowledge of medical terminology and healthcare workflows.

Pay: $23.00 - $26.00 per hour
