About Oben Health
Oben Health is redefining the front door of healthcare by bringing care to where people live, work, and gather. We are building the operating system for decentralized healthcare delivery—powered by mobile connectivity, real-time data, and community-based workflows—to connect patients, CHWs, clinicians, and payers in a scalable system.
The Role
Oben Health seeks an experienced, licensed clinical pharmacist (Pharm.D.) or physician assistant(PA) to serve as a Supervising Provider on an independent contractor basis. In this role, you will function as a key clinical leader, developing and overseeing individualized care plans for a diverse patient population while contributing to the organization's educational mission through curriculum review and development. You will bring clinical depth, cultural humility, and a collaborative spirit to a high-impact role that shapes both patient care and provider education at Oben Health.
Key Responsibilities
Clinical & Health Education for Community Health Workers (CHWs)
- Deliver clinical training content equipping CHWs with the knowledge and skills needed to effectively coach patients on medication adherence, chronic disease self-management, and preventive health behaviors.
- Translate complex pharmacological and clinical concepts into accessible, culturally resonant language and materials suitable for CHW-level education and patient-facing use.
- Refine standardized coaching frameworks, scripts, and reference tools that CHWs can use during patient interactions to reinforce care plan goals.
- Collaborate with CHWs to identify recurring patient barriers — including health literacy gaps, medication concerns, and lifestyle challenges — and co-develop targeted educational responses.
- Provide ongoing clinical guidance and consultation to CHWs navigating complex or escalating patient situations, establishing clear escalation pathways between CHWs and supervising providers.
- Evaluate the effectiveness of CHW-delivered patient coaching through feedback loops, outcome data, and quality metrics; iterate on content and delivery methods accordingly.
- Champion the CHW role as a vital bridge between clinical providers and the communities Oben Health serves, ensuring CHWs feel clinically supported, informed, and confident in their patient interactions.
Patient Care & Clinical Supervision
- Serve as the supervising provider for designated clinical staff, ensuring adherence to scope of practice guidelines, clinical protocols, and applicable state regulations.
- Act as hands-on clinician for initial patient cohorts, managing virtual consultations, remote monitoring data, and hybrid community care pathways.
- Develop, review, and approve individualized care plans for patients representing diverse backgrounds, health literacy levels, and chronic condition burdens.
- Apply a culturally responsive lens to medication therapy management (MTM), including assessment, planning, monitoring, and adjustment of pharmacological and non-pharmacological interventions.
- Collaborate with interdisciplinary care teams for care coordination— including physicians, nurses, social workers, and community health workers — to deliver integrated, whole-person care.
- Conduct or oversee medication reconciliation, clinical screenings, and patient risk stratification across the patient panel.
- Document clinical decisions accurately and in compliance with organizational and regulatory standards.
- Provide mentorship and guidance to junior clinical staff, interns, or students under supervision.
Curriculum Development
- Review existing educational curricula for clinical accuracy, cultural relevance, and alignment with evidence-based practice standards.
- Provide subject-matter expertise to support the development of new training modules, continuing education content, or patient education materials.
- Collaborate with operations leadership to identify educational gaps and recommend improvements.
- Participate in faculty or advisory roles for training programs as needed.
Quality, Compliance
- Ensure all supervised clinical activities comply with federal, state, and organizational regulations, including HIPAA and applicable pharmacy board standards.
- Participate in quality improvement (QI) initiatives, care gap analysis, and population health programs.
- Maintain accurate records of supervisory activities, agreements, and clinical oversight hours.
