Overview
We are seeking a dedicated and compassionate Care Navigator to join our healthcare team. The Care Navigator will play a crucial role in guiding patients through their healthcare journey, ensuring they receive the appropriate care and resources they need. This position requires you to have a care to visit patients homes in Brooklyn, Bronx, Long Island, and other nearby areas. Needs good communication skills, a solid understanding of medical platforms, and the ability to work collaboratively with healthcare nurses, patients, and their families.
Hours
Monday-Friday 9am-5pm
Responsibilities
- Drive to patient's homes to assist patients in navigating the healthcare needs.
- Complete necessary assessments and documentation as needed based on company protocol.
- Educate patients and their care givers about their conditions, treatment options, and available resources to promote informed decision-making.
- Monitor patient progress, follow up on care plans, and complete reassessments following company protocol.
Mandatory Skills
- Bilungual (Spanish/English)
- Proficiency in medical EMRs and understanding of case management principles.
- Experience in care navigation and dementia-focused care.
- Excellent communication skills, both verbal and written, with the ability to convey complex information clearly.
- Ability to work effectively within a multidisciplinary team environment.
- Strong organizational skills with attention to detail for accurate record keeping.

Pay: $25.00 - $30.23 per hour
