Prime Home Health is seeking compassionate and skilled LVNs working on PRN basis to join our in-home health teams in the Spring Branch and Bulverde and surrounding areas. We have many visits available and offer trip charges to offset any longer distances.
You will be visiting adult home health patients and their families. LVNs will be working closely with our interdisciplinary team to ensure the highest quality of care. This is a rewarding opportunity to make a meaningful difference in the lives of patients during a challenging time.
Responsibilities:

You will be providing direct patient care in accordance with the established plan of care and individualized patient needs, including administering medications, performing treatments, monitoring vital signs, and managing symptoms to promote comfort and alleviate pain.
- Care Coordination: Collaborate with the interdisciplinary team, patients and their families.
- Assessments and Documentation: Conduct thorough assessments of patients' condition, document observations, and maintain accurate and up-to-date medical records in Kinnser EHR, including progress notes, medication administration records, and treatment plans.
- Education and Support: Educate patients and their families about their condition, treatment options.
- Safety and Compliance: Adhere to infection control protocols, maintain a safe environment for patients and staff, and comply with all relevant laws, regulations, and organizational policies and procedures.
Qualifications:

Valid LVN license issued by the State of Texas
- Minimum of one year of experience as an LVN in an Adult In-Home Health setting is required.
- Proficiency in using computers and apps such as Kinnser/Wellsky EHR, and Slack for internal communication.
- Current CPR BLS certification
- Strong clinical skills and knowledge of nursing principles and practices.
- Excellent communication and interpersonal skills to interact effectively with patients, families, and the interdisciplinary team.
- Compassion, empathy, and sensitivity to the physical, emotional, and spiritual needs of patients and their families.
- Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic healthcare environment.
- Commitment to maintaining confidentiality and upholding ethical standards in patient care.
