About the Role
Valley Oaks Health is seeking an Integrated Care Therapist to provide brief, evidence-based mental health services within our Federally Qualified Health Center (FQHC). This role works directly in the primary care setting as part of an interdisciplinary team to deliver whole-person, solution-focused care.
Key Responsibilities
- Provide brief, evidence-based therapy (CBT, MI, solution-focused, etc.) within the primary care setting.
- Serve as a core member of the interdisciplinary team, collaborating with primary care providers, nurses, psychiatric staff, and care coordinators.
- Conduct screenings and assessments; develop shared treatment plans addressing both physical and behavioral health needs.
- Support same-day access through warm handoffs and immediate intervention when needed.
- Consult with medical providers on behavioral health needs and social determinants of health.
- Participate in team huddles, case conferences, and population health efforts.
- Track outcomes, monitor progress, and maintain timely, accurate documentation in compliance with HRSA and agency standards.
- Provide crisis assessment and intervention as appropriate in the primary care setting.
Requirements
- Master’s degree in Social Work, Counseling, or related behavioral health field (required).
- Active Indiana licensure (LCSW, LMHC, LMFT) required.
- Minimum of 2 years’ clinical experience, with at least 1 year in a medical or integrated care setting preferred.
- Experience with brief therapeutic models such as CBT, Motivational Interviewing, or solution-focused therapy.

Job Type: Full-time
Pay: $54,080.00 - $71,926.40 per year
