Our team of board-certified physicians, nurse practitioners, and physician assistants works across hospitals, rehabilitation centers, assisted living communities, and even patients’ homes to ensure seamless transitions and compassionate outcomes.
We’re looking for a Nurse Practitioner to function as a Family Medicine/Internal Medicine primary care provider responsible for rounding on patients within a Patient’s Home. This role necessitates close collaboration with the clinical team, administration, supervising physician, home health/hospice team and the patient primary care provider.
Schedule: Monday through Friday
Setting: A Patient’s Home setting involves providing medical care, monitoring, and support services directly in the patient’s place of residence. This environment allows patients to receive ongoing treatment and follow-up care while maintaining comfort and independence at home. The goal of home-based care is to help patients manage their health conditions effectively, avoid unnecessary hospital readmissions and transition them back to their community primary care provider.
Key Responsibilities:
- Provide transition, palliative or long term in home visit services for Geriatric Post-Acute Specialists.
- Conduct in-home assessments including an evaluation of past medical history,review of systems, medication reconciliation, vital signs, and comprehensive physical exam in person and/or virtual
- Working closely with partners in the home setting, ie. Home health, hospice, mobile x-ray, mobile labs, Remote Patient Monitoring, etc.
- Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
- Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
- Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care
- Identify urgent and emergent situations and intervene appropriately
- Educate members on topics such as disease process, medication, and compliance
- Travel up to 90%
- Maintain full active privileges at the facilities where he/she sees patients for Geriatric Post-Acute Specialists.
- Follow Medicare and state guidelines and Employer policy and procedures when providing services and submitting charges into the EHR for billing of services.
- Complete all required documentation for billing and documentation of services rendered by established deadlines.
- Maintain all professional credentials and certifications.
- Attend all business and clinical meetings coordinated by the employer or its vendors.
Required Qualifications:
- Licensures: (Must be current and in good standing)
- Family Nurse Practitioner, Primary Care Nurse Practitioner, Adult Gerontology Nurse Practitioner, Adult Gerontology Clinical Nurse Specialist, Physician Assistant
- Texas License
- DEA
- ACLS/BLS
- Education: Graduate of an accredited Nurse Practitioner program (MSN or DNP) or Physician Assistant program (MPAS or MSPAS)
- Experience: Experience in a patient home setting, preferably including internal medicine, family practice, or post-acute care preferred
- Strong working knowledge of chronic disease management, medication reconciliation, and transitional care
- Proficiency with documentation, coding, and billing practices (Medicare and Medicaid experience strongly preferred)
- Demonstrated ability to work independently while collaborating effectively within an interdisciplinary care team
- Excellent clinical judgment, organization, and communication skills with a compassionate, patient-centered approach
- Ability to round in a patient home setting
*Palliative care experience preferred

Compensation:
Base Salary plus wRVU Bonus
Physical Requirements:
The Nurse Practitioner must be able to perform the essential functions of the job, with or without reasonable accommodation.
