Osborn Health and Rehabilitation is located in Scottsdale, Arizona and we are growing! We would love to grow our team with another MDS coordinator in our fun and fast paced environment. We are located at 3333 N Civic Center Plaza Scottsdale, AZ 85251 and on our website Osbornhealth.com
Duties:
- Oversee and manage health information management for a primary care facility
- Utilize electronic health record systems such as Athenahealth, eClinicalWorks, and Cerner to maintain accurate and up-to-date patient records
- Ensure compliance with industry standards and regulations related to health information management
- Collaborate with healthcare providers to collect and analyze data for quality improvement initiatives, such as HEDIS measures
- Coordinate discharge planning for patients transitioning from hospital to nursing home or hospice care
- Conduct assessments and evaluations to determine appropriate levels of care for patients
- Assist in the development and implementation of care plans for patients
- Provide education and training to staff on documentation requirements and best practices
Skills:
- Strong knowledge of health information management principles and practices
- Proficiency in using electronic health record systems, such as Athenahealth, eClinicalWorks, and Cerner
- Familiarity with industry standards and regulations related to health information management
- Excellent communication and interpersonal skills to collaborate with healthcare providers and staff
- Detail-oriented with the ability to analyze data accurately
- Strong organizational skills to manage multiple tasks and prioritize workload effectively
