We are seeking compassionate, dependable, and highly skilled caregivers to support an individual with complex medical and developmental needs in a one-on-one home setting.

We are currently hiring:
- Registered Nurses (RN)
- Licensed Practical Nurses (LPN)
- State Tested Nursing Assistants (STNA)
- Certified Home Health Aides (HHA)
Candidates with DODD, developmental disabilities, tracheostomy care, mobility assistance, or high-acuity home care experience are strongly encouraged to apply.
Client Care Needs
The individual receiving care has complex physical and medical support needs, including:
- Developmental disabilities
- Muscular disease/limited mobility
- Tracheostomy care needs
- Spinal rod/support complications
- Total or partial transfer assistance
- Specialized positioning and mobility support
Caregivers must be comfortable providing hands-on assistance, following individualized care plans, and maintaining a calm, attentive, and professional approach at all times.
Responsibilities
- Provide direct in-home support and personal care services
- Assist with Activities of Daily Living (ADLs), including bathing, grooming, dressing, toileting, and feeding assistance
- Perform safe transfers and mobility assistance using Hoyer lifts, gait belts, and other adaptive equipment
- Monitor and document patient condition, behaviors, and health changes
- Provide tracheostomy support and respiratory-related care within scope of certification/licensure
- Administer medications and treatments as assigned and permitted by licensure
- Assist with repositioning and injury prevention to maintain client comfort and safety
- Prepare meals and assist with feeding as needed
- Maintain accurate documentation and follow DODD and agency compliance standards
- Communicate effectively with family members, nurses, supervisors, and interdisciplinary care teams
- Maintain a clean, organized, and safe home environment
Qualifications
- Active RN, LPN, STNA, or Certified Home Health Aide credentials preferred
- Experience with tracheostomy care strongly preferred
- Experience working with DODD individuals or developmental disabilities preferred
- CPR and First Aid certification required
- Experience with Hoyer lifts, transfers, and mobility assistance required
- Ability to safely lift, reposition, and physically assist clients
- Strong understanding of infection control and patient safety practices
- Reliable transportation and dependable attendance required
- Professional communication skills and compassionate bedside manner
- Ability to maintain HIPAA confidentiality standards
