Job Description Summary:
Provides personal care and homemaker services for hospice terminally ill patients and their families. Care is provided per the hospice interdisciplinary plan of care to patient in their homes or facilities where they live, independently under indirect nursing supervision. Personal care services include any services related to activities of daily living including bathing, dressing, ambulation/transfer, feeding, incontinence care, positioning and other activities that support patient/family quality of life. Responsible to the primary nurse for carrying out the duties assigned in order to increase the level of comfort and to maintain personal hygiene and safe home environment. Provides homemaking services such as straightening the patient area, washing the patient's clothes and cooking simple meals. Provides temporary respite and relief from total care responsibilities for the primary caregiver and family. Functions as a member of the Hospice interdisciplinary team.

Qualifications High School Diploma required Work Experience 1 year Patient Care experience in a health care setting required Licenses and Certifications Basic Life Support (BLS), Cardiopulmonary Resuscitation (CPR), and Certified Nursing Assistant (CNA) required Essential Functions HOSPICE DOCUMENTATION: Documents care provided Completes documentation per policy in Suncoast Solutions EMR software. Accurately submits mileage documentation. Clearly and accurately documents designated processes, policies, products, service offerings, etc. Ensures that documentation is tailored to expected readers / users. Uses correct terminology. Conforms to required style and format. CARE PROCESSES: Treats patients and families with compassion and implements plan of care. Bases care decisions on plan of care. Monitors and documents patient response. Establishes rapport with clients. Adapts communication style, listens and responds to patient needs. Recognizes age-specific responses to treatment. Provides effective interventions. Respects patients confidentiality and privacy. Supports social, spiritual, and cultural needs of patients and families. Manages difficult or emotional patient situations. HOSPICE CARE: Treats patients and families with compassion and implements the hospice interdisciplinary plan of care Manages and delivers patient supplies to homes. Creating successful interpersonal relationships with patients and families. Communication skills, including the ability to read, write and verbally report clinical information to patients, family caregivers, and other staff. Observation, reporting and documentation of patient status and the care or services furnished. Appropriate and safe techniques in performing personal hygiene and grooming tasks, including bed bath, sponge, tub, shower bath, hair shampoo, nail and skin care, oral hygiene, toileting and elimination, safe transfer techniques and ambulation, normal range of motion and positioning. Asking patients/families what is important to the on an ongoing basis, what brings them peace, comfort, joy. Reading and recording of vital signs. Infection control procedures. Maintaining a safe environment. Adequate nutrition and hydration. Recognizing emergencies and procedures for reporting same. Respecting the physical, emotional, and developmental needs of and ways to work with the populations served by the hospice, including the need for respect for the patient, his or her privacy, and his or her property. Participates in IDT care plan collaboration meetings. Offers quality of life closure activities supporting patients/families experiences. Recognizes, reports and collaborates with IDT regarding suspected abuse, neglect or mishandling of patients. RESPITE CARE: Provides respite care as assigned. Assists with providing respite care for patients. HOME HEALTH PERSONAL CARE SERVICES: Provides personal care to patients in their home Safely provides personal care to clients based on the plan of care. Notifies primary nurse, on-call nurse or Patient Care Coordinator of patient care issues. HOME HEALTH HOMEMAKER SERVICES: Provides homemaker services as assigned. Provides homemaker services such as straightening patient area, assisting with patient laundry, and cooking simple meals. Additional Duties Adheres to the hospital and departmental attendance and punctuality guidelines. Performs all job responsibilities in alignment with the core values, mission and vision of the organization. Performs other duties as required and completes all job functions as per departmental policies and procedures. Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs). Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time. Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills. Wears protective clothing and equipment as appropriate.
