Position Summary
Momentum360 is seeking a compassionate and motivated Certified Peer Support Specialist (CPSS) or Certified Peer and Family Specialist (CPFS/CFPS) to join our outreach team.
This paid position combines peer support, community outreach, behavioral health advocacy, and recovery coaching to empower individuals experiencing homelessness, mental health challenges, substance use disorders, and housing instability.
Using your lived experience and professional training, you'll help individuals navigate complex systems, overcome barriers, connect with resources, and build meaningful recovery goals. This role focuses on relationship-building, mentorship, crisis support, and helping clients stay engaged in services that promote long-term wellness and stability.

Key Responsibilities
Peer Support & Recovery Coaching
- Provide one-on-one peer support and mentorship rooted in lived experience
- Foster hope, resilience, self-advocacy, and recovery through authentic relationship-building
- Support participants in identifying strengths, setting personal goals, and maintaining motivation
- Utilize motivational interviewing techniques to encourage engagement and positive behavioral change
- Offer guidance and encouragement throughout each participant's recovery journey
Community Outreach & Resource Navigation
- Conduct outreach within the community to engage individuals experiencing homelessness or housing instability
- Build trust through consistent, compassionate follow-up
- Connect participants with housing resources, healthcare, behavioral health services, employment, transportation, food resources, and public benefits
- Assist clients in navigating complex systems while advocating for their individual needs
Community Support
- Provide education on behavioral health, addiction recovery, wellness, and available community resources
- Offer guidance, advocacy, and emotional support to family members when appropriate
- Collaborate with community partners, shelters, healthcare providers, treatment programs, and social service agencies
Documentation & Compliance
- Maintain accurate and timely documentation of participant interactions
- Complete case notes and service documentation within electronic health record (EHR) and case management systems
- Track participant engagement, referrals, and outcomes
- Maintain HIPAA compliance and confidentiality standards
Team Collaboration
- Work alongside Case Managers, Outreach Coordinators, and community partners to provide coordinated care
- Participate in case consultations, team meetings, and outreach events
- Represent Momentum360 professionally and compassionately throughout the community
QualificationsRequired
- Active Colorado Certified Peer Support Specialist (CPSS) or Certified Peer and Family Specialist (CPFS/CFPS) credential.
- Lived experience with mental health recovery, substance use recovery, homelessness, housing instability, or family caregiving that informs peer support
- Experience providing peer support, mentoring, recovery coaching, or behavioral health services
- Knowledge of crisis intervention, motivational interviewing, trauma-informed care, and recovery-oriented practices
- Strong communication and relationship-building skills
- Ability to work independently in community-based settings
- Valid Colorado driver's license and reliable transportation
Preferred
- Experience working with individuals experiencing homelessness or housing instability
- Familiarity with Medicaid, behavioral health systems, and community resources throughout the Denver Metro area
- Experience using electronic health record (EHR) or case management software
- Experience with Google Workspace including Google Docs, Google Sheets, and Google Drive
Pay: $22.00 - $26.00 per hour
