Home Health – Field Position
We’re looking for an experienced and compassionate Registered Nurse (RN) to join our dedicated home health team.
Key Responsibilities:
- Utilize OASIS for patient admissions, transfers, re-certifications, and discharges according to physician orders.
- Effectively manage cases and collaborate with LVNs and HHAs to ensure high-quality patient care.
- Maintain full compliance with all agency policies and procedures.
Requirements:
- Active RN license in good standing.
- Minimum of 1 year of home health nursing experience.

PRN- RNs Positions available for multiple market areas :
- Mileage reimbursement
- Bonus Pay for Weekend Visits/ On-call Pay
Per Visit Pay Rates:
- SOC $125 per visit
- ROC $85 per visit
- Discharge $60 per visit
- Regular Visit $50 per visit
- Training/Case Conference/Inservice pay rate $25 per hour
*Sign on Bonus for FT Positions only*
Job Types: Full-time, Part-time, PRN
Pay: $88,000.00 - $90,000.00 per year
