MINIMUM REQUIREMENTS

High School Diploma or G.E.D.

Current CPR, First Aid and AED Certification from an in-person provider

Verbal & written communication skills in English and a demonstrated ability to read and comprehend written/graphic and oral instructions

Competency in pre-algebraic skills for dosage calculations and vital signs conversions
REQUIREMENTS PRIOR TO START DATE

Valid IVP Fingerprint Clearance Card (apply at AZDPS FCC Form Triage)

Proof of immunity to rubeola (measles) and rubella (German measles), or proof of MMR immunization

Bloodborne Pathogen training offered by TUSD Risk Management Department

Must release Social Security Number to be eligible for [AZ Health Care Cost Containment System] AHCCS-Provider ID Number
