Community Care Advocate --- Rochester area --- URGENTLY HIRING
Mon--Fri (with every other weekend included) $17.50--$21.50/hour
We have the possibility of either shift:
- 7:15a-5:15p must be available between these hours // 40 hours a week // every other Sat & Sun on
- 11p-7:30a // 40 hours a week //every other Sat & Sun on
There is shift differential as follows:
- $2.00 an hour for second & third shifts Monday-Friday
- $3.00 an hour for all shifts Saturday-Sunday
Must have 1-3 years of experience working in a Nursing Home and a Clean/Valid Driver License
- Provides a calm, reassuring, and sensitive atmosphere for those with beginning memory loss issues and those advancing through the dementia process.
- Assists the sisters and residents to activity programming, and provides activities designated in each person’s individual activity tracker in times when there is no programming taking place.
- Supports activity and religious programming in group and individualized settings based on interests and support plan.
- Provides access to pastoral care services routinely and at the time of impending death.
- Serves as a personal advocate when someone is afraid or experiencing difficulty.
- Maintains the dignity, right to privacy, and confidentiality of the sisters and residents at all times.
- Assists in extending hospitality to sisters residents, their families, and guests.
- Ensures a “Community” presence for each individual sister as needed.
- Provides support for sisters who are hospitalized.
- Accompanies or ensures that all sisters with memory loss and/or anxiety are accompanied to all medical/dental/diagnostic testing appointments.
- Relates with, schedules, and supports CC Volunteers who assist with meeting the driving, shopping, and social needs of the sisters and residents.
- Facilitates communication with sister’s families, friends, and sisters in the community and communicates on behalf of the sisters as needed.
- Sorts Community Care Department mail: Ensures business mail is forwarded to the Community Care Wellness Manager.
- Delivers sisters’ and residents’ personal mail.
- Ensures correspondence assistance is provided for sisters with memory issues,
Education, Competencies and Experience
To perform the job successfully, an individual should have the following:
- Knowledge of and at least 2 years' experience of gerontology/working in a long-term care setting or equivalent in related field.
- Ability to work independently and as part of a team.
