Part-Time Home Health Registered Nurse – Alexandria, Louisiana
A home health agency in Alexandria, LA is seeking an experienced Part-Time Registered Nurse to join its clinical team.
Requirements
- Active, unrestricted Louisiana RN license
- Previous home health nursing experience required
- Proficiency with Homecare Homebase (HCHB) preferred
- Strong assessment, documentation, and care-coordination skills
- Reliable transportation, valid driver’s license, and automobile insurance
- Ability to work independently while collaborating with physicians and the interdisciplinary team
Responsibilities
- Complete skilled nursing visits and comprehensive patient assessments
- Develop and update individualized plans of care
- Provide medication management, wound care, patient education, and other ordered services
- Document visits accurately and promptly in HCHB
- Communicate changes in patient condition to physicians and agency leadership
- Ensure compliance with Medicare, agency, and regulatory requirements
Schedule: Part-time, based on patient census and agency needs
Location: Alexandria, Louisiana, and surrounding service areas
Pay: $45.00 - $50.00 per hour
