Job Title: Wraparound Coordinator

Reports to: Wraparound Supervisor & Director, Family Services
Job summary: Wraparound Coordinator will consult with the referral source and the family to identify and develop a child and family team. To implement a family centered, strengths based, trauma informed plan that addresses the needs of the child and family while using their natural supports and strengths to reach goals identified by the child and family team while adhering to the National Wraparound Initiative Model. This position is an hourly non-exempt position.
Minimum Job Requirements
- Bachelors’ Degree in Social Work or other Human Services field.
- Previous case management experience in social services and/or child welfare preferred.
- Commitment to the Wraparound principles: Family voice and choice, team based, natural supports, collaboration, community based, culturally competent, individualized, strengths based, unconditional/persistent and outcomes based.
- Excellent problem solving, organization, communication both written & verbal and basic computer skills.
- Ability to plan, organize, and prioritize.
- Pass all pre-employment drug screens, background clearances, and maintain driving clearances.
- Ability to lift 30 pounds, sit, stand, and bend.
- Valid Illinois Driver’s License and vehicle insurance.
- Must be able to work flexible hours to meet the needs of families, including some evenings and occasional weekend hours.
- Required to work in clients’ homes and community settings.
- Required travel to sites throughout identified service area.
- Obtain and document a minimum of fifteen (15) hours of professional development annually.
Duties
- Consult with the referral source and child and family team as identified by the family
- Assist families and their team in identifying, recruiting and engaging natural supports
- Attend child and family team meetings for updates and further planning
- Assist teams in identifying strengths and needs and establishing goals
- Provide leadership in the development, implementation and evaluation of individualized plans using the phases and activities of Wraparound
- Define a safety plan to address behavioral escalations if needed
- Assist the child and family team in preparing a meeting outline/agenda with the family; goals, child/family strengths, concerns, barriers to reaching goals, etc.
- Identify barriers to the process and develop strategies to overcome them
- Ensure the Wraparound process is done with fidelity to the model whenever possible
- Ensure all required confidentiality and other compliance documents are current
- Educate families, providers and the community regarding the Wraparound Process
- Complete all required documentation
- Other duties as assigned
Pay: From $25.64 per hour
