Position Summary
The Home Care Consultant is responsible for developing referral relationships, managing referral opportunities, recovering inactive referral sources, identifying re-admission opportunities, and working closely with our Intake team to maximize qualified patient admissions for our California home health agencies.
This is an office-based position performed remotely from Florida. The majority of the workday consists of phone calls, email communication, CRM management, and collaboration with internal teams. This is not a field marketing or outside sales position.
The ideal candidate enjoys building long-term professional relationships, has exceptional follow-up skills, is highly organized, and takes ownership of every referral opportunity from initial contact through final disposition.
Primary Responsibilities
1. New Referral Development & Conversion
Develop and maintain relationships with referral sources including:
- Physician Offices
- Hospitals
- Skilled Nursing Facilities (SNFs)
- Assisted Living Facilities (ALFs)
- Case Managers
- Discharge Planners
- Referral Coordinators
- Nurse Practitioners
- Physician Assistants
Responsibilities include:
- Receive referral opportunities generated by Community Outreach Representatives and self-generated relationships.
- Contact referral sources promptly.
- Build trust through consistent communication.
- Coordinate closely with Intake to facilitate admissions.
- Monitor referrals from opportunity through final outcome.
- Resolve barriers that may delay patient admission.
- Maintain timely communication with referral partners.
2. Referral Recovery Program
Ensure no referral relationship or opportunity is lost due to lack of follow-up.
Responsibilities include:
- Build and maintain an organized CRM database.
- Contact inactive referral sources regularly.
- Follow up on previously ineligible referrals.
- Identify patients who may now qualify for home health services.
- Maintain scheduled communication with referral partners.
- Document all interactions and future follow-up activities.
3. Re-admission Program
Identify appropriate re-admission opportunities from previously discharged patients.
Responsibilities include:
- Review discharged patient reports.
- Communicate with referral partners regarding potential re-admissions.
- Coordinate with Intake on qualified opportunities.
- Maintain accurate documentation.
- Support continuity of patient care through proactive follow-up.
4. CRM & Pipeline Management
Maintain complete visibility of the referral pipeline.
Responsibilities include:
- Update CRM daily.
- Document every interaction accurately.
- Schedule future follow-up activities.
- Maintain referral source profiles.
- Track referral trends and pipeline activity.
- Ensure every referral opportunity receives appropriate follow-up.
5. Team Collaboration
Work closely with:
- Intake Department
- Clinical Leadership
- Community Outreach Representatives
- Operations Team
- Executive Leadership
Maintain seamless communication throughout the referral-to-admission process while supporting company growth initiatives.

Qualifications
Required Experience
Minimum of two (2) years of experience in one or more of the following:
- Home Health
- Hospice
- Physician Office
- Medical Office
- Referral Coordination
- Healthcare Customer Service
- Patient Coordination
- Healthcare Business Development
- Customer Success
- Healthcare Call Center
Equivalent experience in relationship management, inside sales, or customer success will also be considered.
Education
- Associate degree preferred.
- Bachelor’s degree is a plus.
- Relevant work experience may substitute for formal education.
Technical Skills
- Experience using CRM software.
- Microsoft Office (Word, Excel, Outlook).
- Google Workspace (Gmail, Docs, Sheets, Calendar).
- Strong computer proficiency.
- Ability to quickly learn healthcare software platforms.
Preferred experience with Axxess, ClickUp, HospiceMD, Salesforce, HubSpot, or similar systems.

- Identify and develop appropriate re-admission opportunities.
- Build trusted relationships with healthcare professionals through responsive, professional communication.
- Demonstrate ownership of the referral pipeline and contribute to measurable growth in active referral sources and qualified patient admissions.
