Overview
We are actively seeking a dedicated Long-Term Acute Care Registered Nurse to join our specialized healthcare team. In this vital role, you will provide comprehensive nursing care to patients with complex, ongoing medical needs in a long-term acute care setting. Your expertise will support patient recovery, manage chronic conditions, and facilitate seamless care transitions. Duties
- Deliver high-quality nursing care to patients requiring extended hospitalization due to complex medical conditions, ensuring adherence to established clinical protocols and standards.
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories, utilizing advanced EMR/EHR systems such as Epic or Cerner.
- Administer medications accurately, including IV infusions and injections, while monitoring for adverse reactions and managing acute pain effectively.
- Perform specialized procedures such as catheterization, tube feeding management, wound suturing, and ventilator care with strict aseptic technique.
- Collaborate with multidisciplinary teams—including physicians specializing in critical care, cardiology, ophthalmology, infectious disease, and behavioral health—to develop and implement individualized care plans.
- Engage in discharge planning and coordinate post-acute services such as hospice & palliative medicine or home & community care to ensure continuity of care.
- Maintain comprehensive medical documentation compliant with HIPAA regulations and NCQA standards; accurately code diagnoses using ICD-10/ICD-9 systems for billing and reporting purposes.
- Participate in staff training related to trauma medicine, ICU protocols, trauma & emergency response procedures, and infectious disease control practices.
- Support triage activities in urgent or emergency situations within the facility or during critical transport scenarios.
- Stay current with evolving healthcare practices by engaging in ongoing education related to geriatrics, dementia & Alzheimer's care, developmental disabilities management, and senior care.
Experience
- Minimum of three years of nursing experience in long-term care facilities such as nursing homes or assisted living communities.
- Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks.
- Prior experience working within hospital environments such as Level I or Level II trauma centers is highly desirable.
- Strong background in critical care settings including ICU or emergency room experience; familiarity with telemetry monitoring and ventilator management is essential.
- Knowledge of specialized areas such as dialysis nursing, cardiac catheterization procedures, ostomy & wound care (including suturing), infectious disease management, and pain control strategies.
- Experience working with diverse patient populations including pediatrics (PICU), neonatal (neonatal ventilator), behavioral health cases, individuals with disabilities or developmental challenges.
- Skilled in performing diagnostic evaluations like laboratory specimen collection/processing and sonography when applicable.
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) required; additional certifications related to trauma or pediatric advanced life support are advantageous.
- Familiarity with medical coding (ICD-10/ICD-9), CPT coding for procedures, and compliance with HIPAA regulations ensures accurate documentation and billing processes.
