The Chautauqua County Department of Mental Hygiene & Social Services is seeking a compassionate, organized, and dedicated Case Manager (Adult Protective Services) to join our team in Mayville, NY.
In this rewarding role, you will help vulnerable adults maintain their independence, access essential community resources, manage financial responsibilities, and navigate life's challenges. You'll work directly with adults experiencing mental and/or physical impairments, serving as an advocate while coordinating services that promote safety, stability, and quality of life.

As a Case Manager, you will:
- Conduct home visits and telephone contacts to assess client needs and provide ongoing case management.
- Coordinate services with healthcare providers, social service agencies, housing providers, and community organizations.
- Advocate for clients to ensure they receive appropriate benefits, services, and supports.
- Assist clients with budgeting and financial management, including paying essential monthly bills.
- Serve as a representative payee for benefits when appropriate.
- Help clients complete applications and recertifications for programs such as Medicaid, SNAP, HEAP, Social Security, housing assistance, and other public benefits.
- Assist clients during emergencies, including housing crises, utility shutoffs, and other urgent situations.
- Maintain accurate case notes, activity reports, documentation, and client records in compliance with county, state, and federal regulations.
- Coordinate payments for housing, utilities, and other essential services.
- Arrange supportive services such as transportation, medical appointments, shopping assistance, housekeeping, home repairs, and assisted living placements when needed.
- Participate in regular staff meetings, case reviews, and ongoing professional development.

Minimum Qualifications
Applicants must meet one of the following qualifications:
Option A
- Bachelor's degree from a regionally accredited or New York State registered college or university in a Human Services-related field.
OR
Option B
- Sixty (60) semester credit hours in a Human Services-related field and
- Two (2) years of full-time paid experience providing case management services to adults with mental and/or physical impairments.
OR
Option C
- Four (4) years of full-time paid experience providing case management services to adults with mental and/or physical impairments.
Additional Requirements
- Ability to meet the transportation requirements necessary to perform field visits throughout Chautauqua County.
- Must satisfy New York State driving requirements for this position, including no alcohol- or drug-related driving convictions or refusals to submit to a Blood Alcohol Content (BAC) or chemical test within the three years preceding examination, certification, and appointment.
Civil Service Information
This position is being filled on a provisional basis pending the administration of a future Chautauqua County Civil Service examination. Continued employment will be contingent upon successfully meeting Civil Service requirements.
Pay: $24.42 - $31.51 per hour
