We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our team as a Shift Care Nurse, providing essential nursing care directly in the community. This role offers an exciting opportunity to deliver compassionate, patient-centered care in a home setting. Responsibilities
- Conduct comprehensive patient assessments to evaluate health status and identify care needs.
- Administer medications, including injections and IV infusions, following established protocols and safety standards.
- Perform wound care, catheterizations, tube feedings, and other specialized procedures with precision and aseptic technique.
- Manage medical documentation accurately within electronic health record (EHR) systems such as Epic or Cerner.
- Provide education and coaching to patients and their families on health management, medication adherence, and lifestyle modifications.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans tailored to each individual’s needs.
- Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information.
Qualifications
- Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN)
- Prior experience working in long-term care facilities, assisted living communities, or home health environments.
- Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth for documentation purposes.
- Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
- Skilled in patient assessment, vital signs monitoring, medication administration including injections and IV insertions.
- Experience with specialized care such as Alzheimer’s/dementia/memory care, home health or hospice is a plus.
- Strong understanding of infection control practices and sterile processing procedures.
- Ability to work independently in the field while maintaining excellent communication with healthcare teams. Pay: $31.00 per hour
