FULL-TIME SOCIAL WORK POSITION AVAILABLE IN ASTORIA, NY
New York Center for Rehabilitation and Nursing is a 280 bed skilled nursing and sub-acute rehabilitation facility that serves a diverse population with various medical conditions, cultural backgrounds, and psychosocial needs. Discharge planning for those in our sub-acute program is a critical focus of the position as well as assisting with adjustment issues related to changing life circumstances and serving as an advocate for residents and their families. Collaboration with an interdisciplinary team of medical professionals is a daily occurrence and one that is an enriching and challenging experience.
Qualified applicant will be a motivated self-starter with a Master’s level in Social Work. Bi-lingual candidate with health care experience is preferred.
