Minimum Qualifications/Skills:
Education:
1. Bachelors degree in social work, psychology or similarly related degree program, experience in lieu of degree is also acceptable.
Experience:
1. 1 yr or more of case management experience, preferably with young adults ages 17-24 yrs old
Skills/Certifications:
1. Excellent oral communication, case management skills (including document organization), and ability to empathize and build rapport with young adults.
2. Basic computer skills, including familiarity with Microsoft programs (Word, Excel, Outlook, Teams, etc.).
3. Ability to work flexible hours and attend meetings and training sessions that take place outside of traditional business hours.
4. Ability to organize own time and effort while prioritizing multiple tasks according to immediacy and urgency.
5. Ability to perform conflict resolution or crisis intervention and have excellent communication skills.
6. Ability to use good judgement, demonstrate leadership skills, and serve as a mature role model for young adults.
7. Ability to collaborate effectively with others and work as part of a team.
8. Ability to pass a criminal history background check
9. Ability to drive a medium sized passenger van for client transportation
10. Valid driver's license and clean driving record
11. Knowledge of the TN foster care system, extension of foster care services and child welfare highly preferred!
Essential Job Duties:
1. Adheres to all agency and program policies and procedures and work within the context of
the agency.
2. Demonstrates cultural competence with ability to work with diverse populations within the
community and with agency staff members.
3. Adheres to trauma informed care practice approach and other applicable evidence-based
models in service delivery.
4. Provides coordination or supervision of volunteers or interns as assigned.
5. Works cooperatively and professionally within the agency and with external partners/other agencies, including establishing and using community resources, to enhance service delivery system for clients.
6. Coordinates with RCYC and other staff to implement a supportive transitional living model at all RCYC supervised independent living (SIL) sites
7. Writes monthly and weekly reports documenting the progress of youth participants/clients.
8. Independently and with the RCYC team, guides and provides coaching to young adults in areas of social skills and life skills, including laundry, cleaning, cooking, grocery shopping, budgeting,
job interviewing, and more.
9. Meets in person at least once per week with each assigned client and is available during working hours as needed for clients to share feelings and needs and documents such communications as necessary in case files.
10. Participates in the development and creation of program design and implementation.
11. Participates in an on-call rotation.
12. Screens and responds to placement referrals from DCS in a timely manner (within 24 hours of receiving a referral). Evaluates and assesses eligible youth for participation in the program.
13. Performs the basic administrative tasks related to program and caseload assigned. This includes maintaining client case files of documentations and records, preparation of reports, setting
appointment schedules with clients, and cooperating in a team-based environment with agency staff.
14. Ensures all DCS required information, move notifications, treatment plans, and provider monthly summaries are entered into TFACTS in a timely manner.
15. Maintains a clean and safe supervised independent living home by holding clients accountable to cleanliness guidelines and completing cleaning/organization tasks within the house when necessary
