American Traveler is seeking a Physical Therapist Assistant with home health experience and Homecare Homebase EMR proficiency for a home health role covering York and Lancaster counties in South Carolina.

Home health setting serving patients in York and Lancaster counties, South Carolina

May occasionally be asked to float into Union or Mecklenburg County, though this is not common
-
Patient population is adults only, with diagnoses including chronic diseases, wound care, IV therapy, Pleurx, wound VAC, post-stroke, post-rehab, and post-joint replacements (hip, knee, shoulder)

Expected caseload is approximately 6 points per day with a minimum of 30 points per week

8-hour day shift, 8:00 AM – 5:00 PM, 40 hours per week

No weekends (unless specifically posted as a weekend-only position covering Friday, Saturday, and Sunday)

No on-call requirements

Holiday shifts are available for those who wish to work them

EMR: Homecare Homebase — experience required, no exceptions

Equipment includes Samsung Note 9 phones and a laptop

Active PTA state license required (license required if applicable)

Current BLS certification required

Associate's degree or higher required

Minimum 1 year of recent home health experience required

Homecare Homebase EMR experience required — no exceptions

Experience with adult and geriatric patient populations, chronic disease management, disease teaching, maintenance notes, gait speed assessment, and use of a gait belt

Valid driver's license and proof of auto insurance required

Candidate's permanent address must be located at least 50 miles from the home unit

Last 4 digits of SSN and full date of birth required for consideration

Minimum 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN

PTAs are supervised by a PT within the department
-
Orientation consists of 5 days of new employee onboarding (NEO)
-
First-time travelers are accepted provided they have at least 1 year of home health experience
-
Candidates who have previously worked a permanent position at an AHSE facility must have been separated for at least 1 year before being considered

Light grey scrubs required
-
RTO requests are subject to manager approval

Manager conducts a final clinical interview as part of the selection process
