Employment Type: 1099 Contract / Per Visit
Compensation:$75 per visit
Job Summary:
We are seeking a licensed Physical Therapist (PT) to provide skilled home health services to patients in their homes. The clinician will be responsible for evaluating patients, developing individualized Plans of Care, and delivering evidence-based interventions to improve functional independence and quality of life.
Key Responsibilities:
- Perform initial evaluations and reassessments of patients
- Develop and implement individualized Plans of Care
- Provide skilled therapy services tailored to patient needs
- Supervise COTA/PTA staff as applicable
- Accurately document evaluations, progress notes, and outcomes
- Collaborate with physicians and interdisciplinary care team
- Educate patients and caregivers on treatment plans and safety
- Ensure compliance with Medicare, state, and agency regulations
Qualifications:
- Active state license as a Physical Therapist (PT).
- Minimum of 1 year of home health experience preferred
- Strong clinical decision-making and documentation skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation and valid driver’s license
Preferred Skills:
- Experience with EMR systems
- Knowledge of Medicare guidelines and OASIS documentation
- Bilingual (English/Spanish) is a plus
