Salary: $70,000 (LP) or $72,500 (LMSW/LCSW/LMHC/LMFT)

The Mission
Join our team as a CTI Clinician and provide a lifeline for vulnerable individuals transitioning from hospitals back to community living. Using the Critical Time Intervention (CTI) model, you will provide intensive, time-limited therapeutic support to ensure long-term stability and wellness for those who need it most.
What You’ll Do
- Clinical Care: Deliver evidence-based, solution-focused therapy and conduct mental status and safety assessments.
- Phased Support: Manage an active caseload using the CTI-phased model to bridge the gap between hospital and community.
- Advocacy: Link participants to sustainable long-term resources and collaborate with hospitals and community providers.
- Field Engagement: Provide intensive field-based services across Ulster and Dutchess area
What We Need From You
- Education: Master’s Degree in Social Work, Mental Health Counseling, or a related clinical field.
- Licensure: Valid NYS license (LMSW, LMHC, LCSW, LMFT) or a limited permit.
- Experience: Minimum of 2 years working with individuals with SMI, co-occurring disorders, or I/DD.
- Mobility: Valid NYS Driver’s License (travel required via agency or personal vehicle).
- Skills: Strong clinical competency, cultural humility, and the ability to work independently in a hybrid setting. Spanish-speaking is a plus!
