The Nurse Practitioner (NP) is responsible for providing comprehensive primary and acute medical care to residents within the Ventilator Unit of the Skilled Nursing Facility. The NP practices in accordance with New York State laws and regulations governing Nurse Practitioners and collaborates with the Medical Director, Attending Physicians, Respiratory Therapy, Nursing Leadership, and the interdisciplinary care team to deliver high-quality, evidence-based care to medically complex residents requiring ventilator and tracheostomy management.
The NP promotes quality outcomes by assessing, diagnosing, treating, and managing acute and chronic medical conditions while striving to reduce avoidable hospitalizations, improve resident quality of life, and ensure regulatory compliance.
Essential Duties and Responsibilities
Clinical Care
- Conduct comprehensive initial, routine, and follow-up medical evaluations.
- Perform complete physical examinations and comprehensive medical assessments.
- Diagnose and manage acute and chronic illnesses.
- Develop, implement, and modify individualized treatment plans.
- Prescribe medications, treatments, and durable medical equipment within New York State scope of practice.
- Order, interpret, and evaluate laboratory studies, diagnostic testing, and imaging.
- Perform procedures within the NP scope of practice and facility policy.
Ventilator & Respiratory Management
- Evaluate residents requiring invasive and non-invasive mechanical ventilation.
- Assess ventilator settings, respiratory status, and oxygenation in collaboration with Respiratory Therapy and supervising physicians.
- Manage residents with tracheostomies, chronic respiratory failure, COPD, neuromuscular disorders, and other complex pulmonary conditions.
- Recognize and respond promptly to respiratory deterioration and initiate appropriate interventions.
- Participate in ventilator weaning protocols when appropriate.
- Collaborate with Pulmonology, Respiratory Therapy, and consulting specialists.
Resident Management
- Perform routine rounds on assigned residents.
- Evaluate changes in condition and implement timely interventions.
- Manage admissions, readmissions, and transfers.
- Complete required history and physical examinations and admission documentation.
- Participate in discharge planning and care transitions.
- Provide goals-of-care discussions and advance care planning with residents and families.
Interdisciplinary Collaboration
- Participate in interdisciplinary care plan meetings.
- Communicate effectively with physicians, nursing staff, respiratory therapists, rehabilitation staff, social services, dietitians, and consultants.
- Provide clinical guidance and education to nursing staff regarding resident care plans.
- Collaborate with infection prevention and quality improvement teams.
Quality & Regulatory Compliance
- Maintain accurate, timely, and compliant electronic medical documentation.
- Ensure compliance with CMS, New York State Department of Health, OSHA, HIPAA, and facility policies.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Assist with reducing avoidable hospital readmissions.
- Participate in infection prevention, antibiotic stewardship, and resident safety initiatives.
Emergency Response
- Respond to resident emergencies and rapid changes in condition.
- Evaluate residents requiring emergency transfer.
- Coordinate emergency interventions with physicians and nursing staff.
- Participate in emergency preparedness activities as assigned.
QualificationsEducation
- Master's or Doctoral Degree in Nursing from an accredited Nurse Practitioner program.
Licensure
- Current New York State Registered Nurse license.
- Current New York State Nurse Practitioner certification.
- National Board Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), or Adult-Gerontology Acute Care NP (AGACNP), as appropriate.
- DEA registration and New York State controlled substance registration (if prescribing controlled medications).
- Current BLS certification; ACLS preferred.
Experience
- Minimum of 2 years of Nurse Practitioner experience preferred.
- Experience in skilled nursing, long-term care, subacute rehabilitation, pulmonary medicine, critical care, or hospital medicine strongly preferred.
- Experience caring for ventilator-dependent and tracheostomy residents preferred.
Knowledge, Skills, and Abilities
- Strong assessment and diagnostic skills.
- Knowledge of ventilator management and chronic respiratory disease.
- Ability to recognize early clinical deterioration.
- Excellent communication and interdisciplinary collaboration skills.
- Strong clinical judgment and critical thinking.
- Proficiency with electronic medical records.
- Knowledge of CMS, New York State Department of Health, and long-term care regulations.
- Ability to manage multiple priorities in a fast-paced clinical environment.
- Knowledge of Sigma Care (EMR system)
Physical Requirements
- Ability to stand, walk, bend, and lift up to 25 pounds.
- Ability to respond quickly to resident emergencies.
- Ability to work in a healthcare environment utilizing appropriate infection control precautions.
Reporting Relationship
Reports to the Medical Director and/or Director of Clinical Services, as designated by the facility.
Work Schedule
Full-time, part-time, or per diem as assigned. May require weekend, holiday, evening, or on-call coverage based on facility needs.

Pay: $500.00 - $900.00 per day
