Services for the Developmentally Challenged, Inc. (SDC) is seeking an experienced and detail-oriented Registered Nurse (RN) to serve as a Part-Time Nurse for one of its OPWDD certified group homes. This role is responsible for overseeing all nursing services at the assigned site and ensuring full compliance with regulatory standards across programs serving individuals with intellectual and developmental disabilities (IDD).
The Registered Nurse serves as the clinical authority within the agency, ensuring that all nursing practices, documentation, and medical supports align with requirements set forth by the New York State Office for People With Developmental Disabilities (OPWDD) and New York State Department of Health (DOH).
Key Responsibilities:
Clinical Nursing Services & Compliance
- Provide nursing services to individuals residing in IRA programs in accordance with OPWDD, DOH, and agency policies and procedures.
- Monitor the health status of assigned individuals and ensure timely assessment, intervention, and follow-up care.
- Maintain accurate, complete, and timely clinical documentation, including nursing notes, assessments, and medical records.
- Review Medication Administration Records (MARs) and ensure medication administration practices comply with AMAP regulations and physician orders.
- Participate in incident reviews and provide appropriate nursing follow-up, documentation, and recommendations.
- Assist in maintaining compliance with regulatory requirements and support audit readiness activities.
- PONS: care plans, weekly visits, updating diagnosis, nursing assessments, and other evaluations.
- Other nursing tasks may be assigned as needed to meet organizational goals.
Medical Care Coordination
- Coordinate medical appointments, consultations, and follow-up care with physicians, specialists, pharmacies, and other healthcare providers.
- Review physician orders and ensure implementation of prescribed treatments, medications, and healthcare recommendations.
- Monitor chronic health conditions and communicate changes in health status to appropriate team members and healthcare providers.
- Ensure timely follow-up on hospitalizations, emergency room visits, and significant medical events.
- Participate in interdisciplinary team meetings and contribute to the development and implementation of Life Plans and health-related goals.
Medication Management
- Monitor medication administration systems to ensure accuracy and compliance with regulatory requirements.
- Conduct routine reviews of medication records, physician orders, and treatment documentation.
- Provide guidance and support to AMAP-certified staff regarding medication administration and health-related concerns.
- Report medication errors, adverse reactions, and health concerns according to agency policies.
Training & Support
- Provide health-related education and training to direct support professionals, AMAP staff, individuals served, and family members as appropriate.
- Assist with AMAP training, recertification, and competency validation activities as assigned.
- Serve as a clinical resource to residential staff regarding health care needs, infection control practices, and medical protocols.
- Promote quality improvement initiatives by identifying health trends, concerns, and opportunities for enhanced care.
Quality Assurance & Documentation
- Conduct routine reviews of nursing documentation, health records, and medical tracking systems to ensure compliance and accuracy.
- Assist in the development and implementation of corrective actions related to health services and regulatory findings.
- Maintain confidentiality of all health information in accordance with HIPAA and agency requirements.
- Participate in agency quality assurance activities and continuous improvement efforts related to health care services.
Physical Demands: Requires moderate physical effort on a regular basis.
Working Conditions: A structured residential environment where individuals with developmental disabilities receive assistance in daily activities and routines. It has potential to be a stressful environment due to possible challenging behaviors and other demands of the position. Possible exposure to aggression and possible communicable diseases.
