We are currently recruiting for a Full-Time (Salary) RN Case Manager to serve patients and families in their homes the Pinal County area. (target areas: San Tan Valley, San Tan Heights, neighboring areas and Florence.)

Responsibilities
- Provide compassionate, comprehensive hospice care to patients, visits to home or facility-homes, ensuring comfort and dignity in end-of-life situations.
- Conduct thorough patient assessments and develop individualized care plans in collaboration with interdisciplinary teams.
- Monitor patient progress and adjust care plans as necessary, adhering to HIPAA regulations.
- Educate patients and families about care options, symptom management, and resources available.
- Facilitate discharge planning and coordinate with healthcare providers to ensure continuity of care.
- Maintain accurate medical records and documentation in compliance with medical terminology standards.
